Valid state-issued ID & medical records documenting qualifying conditions from a physician (MD/DO) are required for this type of visit.
If you do not have a paper copy of your medical records, Dr. Forbush will order your medical records. She will send you an Authorization to Release Medical Records. If you have electronic access to your medical records via a secure portal, it may be possible to securely download your information during your visit.
